TCM Pathology of Neck Pain
Neck pain in TCM is often classified as "neck obstruction" (xiang bi), related to wind-cold-damp invasion, blocked meridians, qi-blood stasis or liver-kidney deficiency. Office workers staring at screens maintain chronic neck muscle tension; impaired qi and blood flow can lead to "stiff neck" or chronic neck-shoulder pain.
Western diagnoses such as cervical degeneration or disc herniation can still be addressed through TCM pattern differentiation: wind-cold type from direct air-conditioning; blood stasis with fixed pain and limited movement; liver-kidney deficiency with dizziness, hand numbness and fatigue.

Untreated chronic neck pain may affect shoulder mobility, trigger headaches or arm numbness. Beyond acute relief, TCM also emphasises strengthening qi and blood and freeing meridians to reduce chronic recurrence.
Acupuncture, Tuina and Herbs
Acupuncture commonly uses Fengchi, cervical Jiaji points, Jianjing, Houxi and Hegu to relax sinews, activate collaterals and ease pain. Tuina employs rolling, point pressure and traction techniques to release deep muscles, avoiding severe osteophyte or acute oedema areas. Cupping and gua sha may dispel wind-cold and activate blood.
Internal formulas such as Ge Gen Tang, Qiang Huo Sheng Shi Tang and Xue Fu Zhu Yu Tang are selected by pattern. For recurrent cases, liver-kidney tonifying and sinew-strengthening herbs are added, with neck stretching exercises and ergonomic posture guidance.
Acute stiff neck often shows noticeable improvement within one to three sessions; chronic neck pain may benefit from one to two sessions weekly plus home heat and exercise. Individual results vary.
